Employer Referral Form

Confidential Individual Counselling Support

How this service works

COS Corporate – Workforce Wellbeing provides independent, confidential counselling funded by the employer.

When an employee is formally referred:

  • The employer knows that the employee has been referred and may receive administrative information such as the number of authorised sessions used.
  • The content of counselling remains confidential. COS will not routinely tell the employer what has been discussed, provide therapeutic updates or report whether an employee is “improving”.
  • Confidentiality may only be broken in exceptional circumstances, for example where there is a serious concern about safety, safeguarding or where disclosure is required by law. Where appropriate and possible, this will be discussed with the employee first.
  • Referral does not guarantee that COS counselling will be the appropriate service. The counsellor will assess this with the employee and may recommend alternative or specialist support where necessary.
  • The employee must consent to being contacted by COS.

Confidential Employee Self-Referral

Accessing Your Employer-Funded Counselling

Your employer funds the service.

Your counselling remains confidential.

How anonymous self-referral works

Your employer has arranged access to confidential counselling through COS Corporate – Workforce Wellbeing. You can contact us directly without going through your manager or HR.

Within your organisation's agreed anonymous counselling allowance, we will not tell your employer that you have personally accessed the service.

Your employer may receive anonymous overall figures, for example:

"12 counselling sessions were accessed by employees this month."

They will not be told that one of those employees was you.

If you use your full anonymous allowance and would like your employer to fund further sessions, we will discuss this with you first.  You can then decide whether you want to give COS permission to identify you to the authorised employer contact to request additional funding.

We will never do this without your knowledge and consent.

As with all counselling, confidentiality has limited exceptions where there are serious safety or safeguarding concerns or where disclosure is legally required. Your counsellor will explain this fully when you begin.
